Dementia Care Navigation Services

The Alzheimer’s Association continues to provide the toll-free 24-hour Helpline (800-272-3900), staffed by trained specialists to offer support, address questions and concerns, and to provide crisis services as appropriate. |

Care Navigators, who are licensed social workers and professional counselors, are now available throughout the Chapter to provide care consultations to those diagnosed with Alzheimer’s disease or other dementias and/or their families. The series of interactions consist of: assessment of needs; assistance with planning and problem solving; the development of an action plan; and the provision of support. Other services include providing information and making referrals to community resources; supportive listening and follow up. There is a fee for this service which is based on a sliding fee scale.
