Dementia Care Navigation Services
Dementia Care Navigation Services: Supporting you and your loved ones by creating a road map for navigating your journey.
Care Navigators, who are licensed social workers and professional counselors, are now available throughout the Chapter to provide care consultations to those diagnosed with Alzheimer’s disease or other dementias and/or their families. The series of interactions consist of: assessment of needs; assistance with planning and problem solving; the development of an action plan; and the provision of support. Other services include providing information and making referrals to community resources; supportive listening and follow up. There is a fee for this service which is based on a sliding fee scale.

The Alzheimer’s Association continues to provide the toll-free 24-hour Helpline (800-272-3900), staffed by trained specialists to offer support, address questions and concerns, and to provide crisis services as appropriate.
