Description:
For people with Alzheimer’s disease, a planned and unplanned hospitalization often results in increased confusion, risk for falls, use of restraints, deconditioning, increased length of stay and financial costs. What can hospital staff and family members do to prevent or reduce these consequences? What essential information should nursing home staff communicate before transfer to the emergency room? Join us and learn how to:
Objectives: 1. Anticipate illness or medical conditions that may not require hospitalization. 2. Reduce anxiety and confusion without chemical or physical restraint. 3. Manage pain more effectively. 4. Identify discharge needs more accurately and facilitate family caregiver instruction.

Description:
Person-centered care is a social approach to caregiving that is increasingly being marketed by healthcare organizations. What does it really mean in practice for both caregivers and people with dementia? What knowledge and skills do caregivers need to implement this approach in everyday interactions with people in their care? What kind of supports do caregivers need from their managers and administration to deliver person-centered care? This seminar will focus on activities of daily living such as eating, bathing, dressing and toileting in order to answer these important questions. At the end of this seminar, participants will be able to: Define person-centered care. Differentiate between the traditional medical model of care and a social model of care. Implement person-centered care in everyday care tasks.
