The Alzheimer’s Association Illinois Chapter is now taking speaker requests. Requests may be made for a physician, local researcher, or members of our Illinois Chapter medical and scientific committee.
The Alzheimer’s Association is a nonprofit organization. We work hard to serve individuals and families impacted by Alzheimer’s disease and other dementias and raise awareness in the community. The vast majority of our direct services to families are provided free of charge. To help us keep those services free, we may ask organizations requesting a speaker to pay a small fee to help defray the costs of staff time, travel and print educational materials distributed at the speaker event. We will let you know if fees apply and what the exact fee is when we respond to your request.

PLEASE NOTE:
Please request presentations at least 8-12 weeks in advance. A speaker is generally not confirmed until 6 weeks prior to the speaker event, unless otherwise requested on the form.
You are responsible for marketing, publicizing and registration for this event.
A MINIMUM OF 15 PEOPLE IS REQUIRED TO ATTEND THE SPEAKER EVENT. This must be confirmed via phone or email prior to 48 hours of the speaker event, otherwise the speaker event is subject to cancellation. We ask that you collect RSVPs for the speaker event, and also send reminders to participants 48 hours in advance. We reserve the right to charge an agreed-upon fee in the case of an audience not meeting the minimum of 15 attendees; we may hold your credit card information as a security deposit.
Due to limited resources, we are not able to accommodate all speaker requests. Your request is not final without a signed confirmation agreement between the Alzheimer’s Association and requester.
